Registration

Please bring the following documents with you to register.

• Proof of Residency (Power bill, property tax bill, proof of purchase, etc. are examples of acceptable proof of residency)
• Certified Birth Certificate
• Immunization Records on GA Form 3231
• Form 3300 - Certificate of Vision, Hearing, Dental, and Nutrition Screening (Click here for Form 3300) (To be completed by a physician)
• Student Social Security Card 
• Drivers License or State Issued ID of enrolling parent/guardian
